A cataract is a clouding of the natural lens inside your eye. The lens sits behind the iris and is normally clear, focusing light onto the retina to produce sharp vision. As a cataract develops, vision becomes blurred, hazy, or washed out; colours may seem faded; and glare and haloes around lights become more noticeable. Cataracts are most often a natural consequence of ageing, though they can also develop following eye injury, prolonged steroid use, or conditions such as diabetes. Surgery is recommended when the cataract is affecting your quality of life to a degree that matters to you. There is no single threshold; the decision is based on how much your vision is limiting everyday activities such as reading, driving, or recognising faces. In some cases an updated spectacle prescription can temporarily improve vision, but once a cataract has progressed beyond a certain point, surgery is the only effective treatment.

NHS cataract surgery is an excellent, safe procedure, but waiting times can be significant and the lens used is a standard monofocal implant, which corrects distance vision but typically means you will still need reading glasses afterwards. During cataract surgery, your natural cloudy lens is removed and replaced with a clear artificial intraocular lens (IOL). In private practice, you have a choice of lens types. A standard monofocal lens corrects vision at one distance (usually distance), so reading glasses are still needed. A toric monofocal lens additionally corrects astigmatism, giving sharper distance vision without spectacles. Extended depth-of-focus (EDOF) lenses provide a continuous range of vision from distance to intermediate with fewer side effects than multifocal lenses. Multifocal lenses aim to provide vision at distance, intermediate, and near, potentially eliminating the need for glasses altogether in suitable patients. Premium lenses are available only in the private sector; the NHS funds surgery using a standard monofocal lens only. Whether you still need glasses depends on the lens chosen and your individual eyes. With a standard monofocal, most patients still need reading glasses. With EDOF or multifocal lenses, many patients achieve functional vision across distances without glasses, though a small residual prescription is possible. Your initial consultation is a comprehensive assessment including a full history, measurement of your vision and spectacle prescription, slit-lamp examination of the front and back of your eyes, assessment of the cataract, and biometry to calculate the correct lens power. Miss Ali will explain her findings clearly and ensure you feel fully informed before making any decisions. There is never any pressure to proceed with surgery. Regarding other eye conditions: previous LASIK, PRK, or LASEK requires specialised biometry calculations and careful lens selection, as standard formulas are less accurate in post-laser eyes. Dry eye disease needs to be assessed and optimised before surgery, as it affects biometry accuracy and the suitability of certain lens types. Macular degeneration does not exclude you from surgery, but the expected visual improvement depends on the severity of the macular condition, and premium multifocal lenses are generally avoided in these patients. Cataract surgery is performed as a day case under local anaesthetic, meaning you are awake but your eye is completely numb and you feel no pain. You will be at the hospital for approximately two to three hours in total, though the operation itself takes around 20 to 30 minutes. Anaesthetic eye drops are administered beforehand, and the surgeon makes a tiny self-sealing incision to remove the cloudy lens and implant the new IOL. You will need someone to drive you home. You will be given antibiotic and anti-inflammatory eye drops to use for several weeks after surgery.

Most patients notice a significant improvement in their vision within 24 to 48 hours of surgery, though the eye continues to settle over the following weeks. Vision may be slightly blurred or hazy immediately after the procedure, and colours often appear noticeably brighter once the cloudy lens has been removed. Full stabilisation of vision typically takes four to six weeks. You should not drive on the day of surgery. Most patients are able to drive again within a few days, once their vision has stabilised sufficiently to meet the DVLA standard (reading a number plate at 20 metres); Miss Ali will confirm this at your post-operative review. Many patients return to desk-based work within a few days. If your work involves physical exertion, dusty environments, or exposure to chemicals, you may need to wait one to two weeks. Swimming should be avoided for at least four weeks to reduce the risk of infection.

Posterior capsule opacification (PCO), sometimes called a secondary cataract, is a common development that occurs in some patients months to years after cataract surgery. The membrane behind the lens implant gradually becomes cloudy, causing vision to deteriorate in a way that can feel similar to the original cataract. It is not a problem with the lens itself and does not mean the surgery has failed. PCO is treated quickly and painlessly with a YAG laser capsulotomy, a brief outpatient procedure that takes only a few minutes and restores clear vision immediately.

Refractive lens exchange (RLE), also called clear lens extraction or lens replacement surgery, is the same surgical procedure as cataract surgery but performed before a visually significant cataract has developed. The motivation is refractive: to correct a prescription and reduce dependence on glasses or contact lenses. RLE is most commonly considered in patients over 50 with high prescriptions or presbyopia (age-related loss of near focus) who are not suitable for laser eye surgery.
